EXHIBIT “A”
INVITATION FOR BID (IFB) #035-25
PRINTING & STATIONERY SPECIFICATIONS
SECTION ONE ~ BUSINESS CARDS AND STATIONERY
1) Standard County Business Cards:
The cards are to be printed on Royal Brilliant White Linen Stock, 80#, with the following colors:
Pantone 505, Pantone 357, and Pantone Black, flat print only. The cards are single-sided. Card size is 3½” x 2”.
2) Sheriff’s Office Business Cards:
The cards are to be printed on Kromekote C1S (coated 1 side) 10pt cover. The cards are printed 2 /
1. The cards are double-sided. The front of the card is printed with a 2-color process, PMS 303 &
872. The back of the card is printed with 1 color, PMS 303 Blue. Card size is 3½” x 2”.
3) Coroner’s Office Business Cards:
The cards are to be printed on Royal Fiber Brilliant White 80# Cover. The cards are printed 4 / 0 using 4 spot colors, Black & PMS 340, 464 & 872 flat print only. Card size is 3½” x 2”.
4) District Attorney’s Office Business Cards:
The cards are to be printed on Cover Stock 100#. The cards are single-sided. Black and White. Card size is 3½” x 2”.
5) Public Health Department Business Cards:
The cards are to be printed on Satin Cover 130#, with the following colors: Pantone 505, Pantone 357, and Pantone Black. The cards are single-sided. Card size is 3½” x 2”.
6) Standard County Letterhead:
The letterhead is to be printed on Royal Brilliant White Linen, 24#, 8½” x 11”. The following colors will be used on the letterhead: Pantone 505, Pantone 357, and Pantone Black, flat print only.
Standard packaging of five hundred (500) sheets per box.
7) Sheriff’s Office Letterhead:
The letterhead is to be printed on Royal Brilliant White Linen, 24#, 8½” x 11”. The following colors will be used on the letterhead: PMS 872 Gold and PMS 303 Blue. Due to the tight design, registration, screens, negatives, and metal plates should be used. Standard packaging of five hundred
(500) sheets per box.
8) Coroner’s Office Letterhead:
The letterhead is to be printed on Nekoosa Bright White 24# Linen Writing 8½” x 11”. The letterhead is printed flat using 5 spot colors, Black, PMS 340, 464, 872, with PMS warm gray #11 screened 5% on the front of the letterhead. Standard packaging of five hundred (500) sheets per box.
9) Standard County Letterhead and Sheriff’s Office Letterhead Second Sheets:
The second sheets are to be Royal Brilliant White Linen, 24#, 8½” x 11”. Standard packaging of five hundred (500) sheets per box. No printing is required.
SECTION ONE ~ BUSINESS CARDS AND STATIONERY
10) Coroner’s Office Letterhead Second Sheets:
The second sheets are to be Nekoosa Bright White 24# Linen Writing 8½” x 11”. The second sheets are printed with PMS warm gray #11 screened 5% on the front of the letterhead. Standard packaging of five hundred (500) sheets per box.
11) Standard County Letterhead Envelopes:
The letterhead envelopes are to be Royal Brilliant White Linen, Number 10 / 24 Writing. The following colors will be used on the envelopes: Pantone 505, Pantone 357, and Pantone Black, flat print only. Standard packaging of five hundred (500) envelopes per box.
12) Sheriff’s Office Letterhead Envelopes:
The letterhead envelopes are to be Royal Brilliant White Linen, Number 10 / 24 Writing. The following colors will be used on the envelopes: PMS 872 Gold and PMS 303 Blue. Due to the tight design, registration, screens, negatives, and metal plates should be used. Standard packaging of five hundred (500) envelopes per box.
13) Coroner’s Office Letterhead Envelopes:
The letterhead envelopes are to be Nekoosa Bright White Linen, Number 10 / 24. The envelopes are printed with 4 spot colors, Black, PMS 340, 464 & 872. Standard packaging of five hundred (500) envelopes per box.
14) Public Health Department Envelope With Window:
The letterhead envelopes are to be White Wove, Number 10 / 24 Writing, peel and seal, window 4½” x 1-1/8” on the left-hand side of envelope. The following colors will be used on the envelopes:
PMS 357 and PMS 505. Standard packaging of five hundred (500) envelopes per box.
15) Public Health Department Envelope Without Window:
The letterhead envelopes are to be White Wove, Number 10 / 24 Writing, peel and seal. The following colors will be used on the envelopes: PMS 357 and PMS 505. Standard packaging of five hundred (500) envelopes per box.
16) Sheriff’s Office Note Cards & Matching Envelopes:
The note cards and matching envelopes are to be on a high-quality white card stock, 110#. Cards are scored, 8½” (scored to fold down to 4¼”) x 5½” with die cut for a business card. The following colors will be used on both the note card and envelope: PMS 872 Gold and PMS 303 Blue. Due to the tight design, registration, screens, negatives and metal plates should be used. Standard packaging of one hundred (100) note cards and envelopes per box.
SECTION TWO ~ FORMS (SINGLE SHEET AND MULTIPLE PART)
NOTE: ALL NCR FORMS MUST BE PRINTED ON 19# OR 20# STOCK
1) Form, two-part standard sequence, NCR, 8½” x 5½”, black print, print on one side only.
2) Form, two-part standard sequence, NCR, 8½” x 11”, black print, print on one side only.
3) Form, two-part standard sequence, NCR, 8½” x 11”, black print, print on both sides.
4) Form, two-part standard sequence, NCR, 8½” x 11”, two-color print, print on one side only.

13) Investigator’s Note Pads:
Pad of single sheet forms, 60#, 8½” x 11”, black print on canary-color paper, print on one-side only. Three-holes drilled on the left side of the form and two-holes drilled on the top of the form (hole measurements are standard to fit into a 3-ring binder or classification folder). Each pad includes twenty-five (25) individual sheets.
SECTION TWO ~ FORMS (SINGLE SHEET AND MULTIPLE PART)
14) Field Interview Card:
Pad of single sheet forms, 5” x 3”, black print on 110# white card stock, print on both sides. Each pad includes fifty (50) individual cards.
15) Douglas County Sheriff’s Office Evidence/Property Form:
Form, three-part, NCR, 8½” x 11”, black print on white paper. First page is double-sided, pages two and three have print on one-side only. All three pages are on white paper.
16) Abandoned Vehicle Report Hang Tag:
Tag, 110# yellow card-stock, 3” x 6¼” with a 14½” top reinforced hole string attachment. Black print, print on one-side only.
17) Domestic Violence Forms:
Form, five-part, NCR, 8½” x 11”, black print (double-sided) on white paper. Padded at top. All five pages are on white paper.
18) Driver’s Exchange of Information Form:
Form, five-part, NCR, white, green, yellow, pink and gold, black print 2 sides, 5½” x 8½”, padded at top.
19) Post-it Notes:
4” x 6” on white stock, full color, fifty (50) sheets per pad.
SECTION THREE ~ ENVELOPES & MISCELLANEOUS PROJECTS

10” x 15”, 28# White Wove Envelope.
10” x 15”, 28# Grey Wove Envelope.
8) Graph Pads:
8½” x 11”, one-hundred (100) sheets per pad, white 60# Accent Opaque VIP Smoot Digital, printed two colors front in black & blue ink. Padding one-hundred (100) sheets per pad USE CHIPBOARD THAT IS LONG GRAIN. Douglas County Logo, text lines, date, and page number on the top of the paper; address and telephone number on the bottom of the paper. One annual delivery of one-hundred (100) pads.
9) Bicycle Maps:
Map, 36” x 24”, full color, 2 sided, 70# matte book paper, trim and accordion fold to 4” x 24” and refold into thirds to finished size of 4” x 8”. One annual delivery of approximately 6,000 maps.
