About this file

This is a Performance Requirements Summary exhibit that details the quality control standards and requirements for a pest control services contract at Fort Riley, KS. The document outlines four key service areas with their associated performance standards, acceptable quality levels (AQL), surveillance methods, and proportion of total contract price.

The required services include: 1) Pesticide Usage Reporting Forms with 5% AQL and 2% of contract value, requiring complete documentation of all pesticides used; 2) Scheduled Services and Service Orders, both with 1% AQL and 5% of contract value each, requiring pest control according to Quality Control Standards; and 3) Wood Destroying Insects (termites) Treatments with 1% AQL and 10% of contract value, requiring standard industry techniques and a five-year warranty against reinfestation. The surveillance methods range from random sampling and planned inspections to 100% inspection for termite treatments, with Quality Assurance Evaluator (QAE) reviews, customer comments, and on-the-spot inspections used to verify compliance.
