INSTRUCTIONS:
1. List all areas given in Tables A-2 and A-4 in the order they are to be cleaned.
2. In the boxes to the right of the services listed, enter M, T, W, Th, F, Sa, and/or S, for work performed at a weekly frequency or greater.
For work performed at less than a weekly frequency, i.e., monthly, enter M, T, W, Th, F, Sa, or S, preceded by a number to indicate the day of the month, i.e., the second Tuesday of the month would be 2T. Precede with an asterisk (*) to indicate bimonthly frequency, i.e., *2T.
3. On the Completion Times, give the time (ex. 30 minutes) that work will be completed in each public use area.
