EXHIBIT 3 - Inspection Reports Summary
This document comprises underwater inspection reports prepared by Underwater Engineering Services, Inc. for the St. Johns River Water Management District (SJRWMD), detailing structural assessments of eighteen water control structures located throughout Florida's flood control system. The inspection reports document findings from dive operations conducted between April 21, 2025, and July 8, 2025, covering gated culverts (AP-1, B-1, E-2, E-3, S-2), pump stations (PS-2N, PS-2RO), and associated infrastructure. Each inspection was performed by five-person dive teams utilizing surface-supplied air and comprehensive underwater documentation. The reports identify specific deficiencies rated on a six-tier condition scale (C-1 through C-6), with recommended actions ranging from monitoring to immediate repair/replacement. Critical findings include corroded barrel clamp bolts with up to 50% section loss (AP-1, B-1), cracked pipe liners requiring sealant application (B-1, E-3), exposed filter fabric with riprap displacement (E-2, E-3), gate seal damage and missing seals (AP-1, E-3, S-2), sheet pile wall corrosion (PS-2N), flap gate hinge corrosion and missing washers (PS-2RO), and significant sediment accumulation in multiple barrels (B-1, E-2, E-3, S-2). Inspections documented underwater visibility ranging from 1 to 3 feet and assessed structural conditions of access piers, concrete saddles, culvert barrels, gates, seals, erosion control features, and associated appurtenances.
The inspection ratings indicate no items requiring immediate structural repair (C-5 rating) across all eighteen structures; however, numerous items requiring routine repair/replacement (C-3 rating) and monitoring (C-2 rating) were identified. Recommended corrective actions include replacement of barrel clamp bolts with stainless steel hardware, application of epoxy coatings to corroded steel elements, installation of flexible sealants in cracked liners, deployment of riprap in scour holes, removal of sediment accumulation, replacement of corroded hinge hardware, installation of lubricating grease in gate mechanisms, verification of staff gauge elevations, and monitoring of advanced corrosion, decay, and structural degradation. The reports provide photographic documentation, detailed checklists utilizing standardized inspection item categories, depth measurements of damage, probable causes of deficiencies, and prioritized repair sequencing. All inspections were conducted under contract 40758 and submitted under the professional seal of Jeffrey O'Connor, P.E. (Florida License No. 50914), with verification required on electronic copies.

Inspection Date: 6/10/2025 E2 – Page 3 of 10
Structure E2 is a gated culvert comprised of three barrels, extending southeast (gated end) to northeast under Levee E-2. The barrels are CAP, 60 inches diameter by 80 feet long. The upstream end has an access pier comprised of timber piles, a timber superstructure, and steel channels for cross bracing. The east and middle barrels have a gate at the barrel ends. The west barrel has a riser with two gates mounted to the riser.
The underwater observations were performed by a 5-person dive team. The underwater structural inspection was conducted on 6/10/2025. The dive team worked from a dive trailer, using surface-supplied air, and accessed the structure areas from the bank. The upstream waterway has surface vegetation, numerous baby alligators, and an adult alligator that could not be caught. For safety reasons, the upstream side was inspected topside, with estimated less than 2’ of structure being underwater at the time of inspection.
The scope of services included the underwater inspection of the submerged structure components. The area extended 20 feet beyond the structure edges. There are no boat barriers.
The air temperature was 85 degrees F. and the weather was partly cloudy. There was no diving as described above on the upstream side, and the downstream barrels were dry. The upstream water level was approximately 2.1 Ft per the U/S staff gauge.

Rating: C-3 Photo Description:
X-Bracing corrosion and pile cap damage
Deficiency: (1) The cross bracing typically has 100% coverage of corrosion with rust scale. (2) The western two boards for the south side pile cap beam on the 5th bent from the road has cracks and section loss on the bottom with 1" to 3" of loss on the bottom (inset photo).
Probable Cause: (1) The steel channels do not appear to have any initial coating. Corrosion is caused by the wet environment and lack of coating. The stainless steel bolts are causing galvanic corrosion to the channels. (2) The section loss and cracks on the pile cap beam may have been caused by decay or knots in the board.
Recommendation: (1) Clean and recoat the steel channel cross bracing. Place plastic washers under the bolt heads and nuts to separate the stainless steel bolts from the mild steel channels. (2) Replace the two western boards of the south side pile cap beam on the 5th bent from the road.
