EXHIBIT 3
PB 100 - Gravel Crushing
Task Statement and Quality Standards
PB 100.01 TASK STATEMENT
The contractor shall furnish all labor, equipment, material, and incidentals necessary to produce and stockpile crushed gravel and crushed stone at the designated location, in the quantities, quality and gradings specified.
PB 100.02 QUALITY STANDARDS
1. The crushed materials conform to the required gradation and plasticity index specifications.
2. Suitable topsoil was stripped and stockpiled as designated by the Contracting Officer’s
Representative.
3. Unsuitable material was stockpiled at designated locations.
4. Oversize material was reduced and utilized in the production of aggregate.
5. Development and stockpile areas were sloped to drain.
6. All drainages were protected from sedimentation and blockage.
7. The working face of the development area conformed to MSHA regulations during crushing.
8. All created or substantially modified side slopes of the development area were left at a slope of
2:1 (H:V) or flatter at the completion of the contract.
9. The pit area was resloped following crushing as noted on the respective pit development plans.
10. Existing stockpiles were not disturbed unless approved by the Contracting Officer’s
Representative.
11. A buffer of twelve feet was maintained between existing and new stockpiles.
12. All disturbed areas were seeded with the specified seed mix if required.
13. A 12-foot wide trail, passable by a crawler tractor was constructed and maintained at the top of the cut along the developed area.
14. Any filler sources developed were rehabilitated and seeded.
15. Access roads to the quarry and filler source locations were maintained.
PB 100.03 MEASUREMENT
The quantity to be paid for will be the weight of the material measured by a conveyor scale that shall meet the requirements specified below. The contractor will also be required to furnish a certified scale receipt for a minimum of one truckload of the crushed material in order to verify the accuracy of the belt scale. The Contracting Officer’s Representative may require additional loads. Costs associated with weighing the crushed material are included in all pay items and no additional payment will be made. The tonnage weight shall not be adjusted for moisture content.
The contractor shall use a belt conveyor scale that meets the design, marking, installation and tolerance requirements of the National Bureau of Standards (NBS) Handbook No. 44. The contractor shall provide a copy of a NBS Prototype Examination Report of Test to certify the scale.
The contractor shall use a weighing mechanism that contains a weight totalizer and a self-printing device that legibly imprints the load-out weight on appropriate serially numbered and time-dated tickets. Time date manually or use an automatic printing device. Deliver each ticket at the job site or point of use.
Furnish the totalizer calibration adjustment and ticket-imprinting device with a security lock and key.
Under the observation of the contracting officer’s representative, the contractor shall run a daily zero-load test in accordance with the NBS Handbook No. 44.
PB 100.04 PAYMENT
The accepted quantities will be paid for at the contract unit price for each pay item shown in the
SCHEDULE OF ITEMS.
