About this file
This document is an addendum to the standard instructions to offerors (FAR 52.212-1) for a Request for Quotation (RFQ) issued by the Defense Health Agency for the Naval Medical Center Portsmouth Pathway to Excellence Program Designation. The RFQ seeks a single contractor to provide organizational credentials recognizing healthcare facilities for creating positive work environments where nurses can flourish, equivalent to credentials provided by the American Nurses Credentialing Center Pathway to Excellence Program. The contract will be Firm Fixed-Price for commercial, non-personal services with a one-year base period and three one-year option periods. All quotations must be submitted electronically via email to Matthew Teel, Contracting Officer, at matthew.d.teel.civ@health.mil by the deadline specified on the RFQ cover page, with pricing valid for a minimum of 90 calendar days from submission.
Offerors must provide legible, comprehensive, and factual responses including technical capability documentation (limited to three pages maximum) that clearly maps how they meet each requirement in the Performance Work Statement, along with separate pricing for all identified Contract Line-Item Numbers (CLINs 0001-3001). All submissions must be in searchable Adobe PDF format using Times New Roman font (12 pitch for narrative, 10 pitch for exhibits), compatible with Adobe Acrobat Reader 25 and Microsoft 365. Critical submission requirements include contractor registration in the System for Award Management (SAM) with a Unique Entity number and CAGE Code, completion of FAR 52.212-3 representations and certifications, and confirmation of receipt by the contracting officer. Quotations submitted by mail, hand-carry, or through proprietary file transfer methods will not be evaluated and will render submissions unacceptable.
