A. 52.212-2 EVALUATION—SIMPLIFIED ACQUISITION

PROCEDURES

The following provisions are incorporated into 52.212-2 as an addendum to this solicitation:

Basis for Award. The Government intends on issuing a Firmed Fixed Price to one responsible offeror whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

(a) The following factors shall be used to evaluate quotations:

(1) Technical

(2) Past Performance

(3) Socioeconomic Status (SDVOSB)

(4) Price

(b) Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s).

Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider the following:

Factor 1 Technical:

The quotation will be evaluated to the extent to which it can meet and/or exceed the Government’s requirements as outlined in the solicitation and based on the information requested in the Instructions to Offerors section of the solicitation.>

a) The Government will evaluate quotes in the same order as presented herein. Quotes shall be organized with sections appropriately identified in paragraph (c) of Technical Factors. Quoters are to provide a narrative on how they intend to fulfill the requirements of this solicitation and how their total submission will meet the minimum needs of the specifications while demonstrating a full understanding of the Scope of Work. The quote should be concise and provide sufficient information to demonstrate the capability to successfully perform the tasks outlined in the Performance Work Statement.

b) Quoters shall provide their best quote, as the government intends to award without discussions.

c) Quotes shall be organized as follows:

1.) Section 1 – Cover Letter. The cover letter shall be a maximum of two-pages and shall include the name, address and DUNS# of the organization submitting the quote, along with the name, address, and telephone number of the contact person who has the actual authority to bind the contractor and make representations relative to the quote and any resultant contract for the organization.

2.) Table of Contents. The Table of Contents shall be detailed to include sequential page numbering, Section Reference and Section Title. The table of contents shall also include an outline of the contractor’s quotations.

d) Proposed Personnel:

1.) Offeror shall provide experience with processing Medicaid application

2.) Offeror shall include knowledge of South Carolina and Georgia laws/procedures of processing Medicaid Applications

3.) Offeror shall include knowledge of HIPPA, Release of Information Policy, Privacy, Computer Security, Medicaid, TMS, Quality Assurance Plan.

Factor 2 Past Performance:

Past performance evaluations will be conducted using information provided with the quotes, information obtained from references, information obtained from the Past Performance Information Retrieval System (PPIRS), and information from any other sources deemed appropriate. When evaluating past performance, the government will consider the currency and relevancy of the information, the source of the information, the context of the data provided, and the general trends in the contractor’s performance. The government will also take into account past performance information regarding predecessor company’s key personnel who have relevant experience and/or subcontractors that will perform major or critical aspects of the requirement. Quoters without available relevant past performance history will not be evaluated favorably or unfavorably. Quoters are cautioned that the Government may review available past performance data available in the PPIRS. The Government reserves the right to obtain past performance information from any available source and may contact customers other than those identified by the contractor when evaluating past performance. Since the government may not necessarily interview all of the provided Sources, it is incumbent upon the contractor to explain the relevance of the data provided.

Contractors are reminded that the burden of proving low performance risk rests with them.

a.) Offerors should have experience with similar size contracts and scope of work.

Contractors shall provide examples of similar services provided and what agency the services were provide for.

Factor 3 Socioeconomic Status:

In accordance with Veterans Affairs Acquisition Regulation (VAAR) 852.215-70 (DEVIATION), Service-Disabled Veteran-Owned Evaluation Factors, the Government will assign evaluation credit for a Quoter (i.e., prime contractor) which is a Service- Disabled Veteran-Owned Small Business (SDVOSB). To receive credit, an offeror must be registered and verified in Vendor Information Pages (VIP) database at time of quotation submission and at time of award ( https://www.vip.vetbiz.va.gov/) and must meet federal small business size standards for the North American Industry Classification System (NAICS) code assigned to this solicitation.

Factor 4 Price:

The Government will evaluate the price by adding the total of all line item prices, including all options The Total Evaluated Price will be that sum.

a.) Pricing shall be provided on a unite prices basis as noted in the solicitation. The completed and signed solicitation shall be returned as part of the offer. Contractors may submit a quote for one, multiple or all facilities, but must submit a price on all of the requirements (i.e., line items) for each facility, for which a quote is submitted, with a final estimated total price. Pricing should be an all-inclusive rate when considering any travel.

b.) Period of acceptance of Quotes: The Contractor agrees to hold the quote for 90 calendar days for specified date for receipt of quotes as stated on the SF1449 Block 8, unless another time period is specified in an addendum to the solicitation.

c.) Evaluation of price will be made on the total performance period (including base and all option years) and must also be determined to be fair and reasonable. Quoters are cautioned that an unrealistically low price or materially unbalanced pricing may be grounds for eliminating the quote on the basis that the quoter does not fully understand the requirement.

d.) “Evaluations of Options"

For purposes of award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be those rates in effect under the contract each time an option is exercised under this clause. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
