1. Per the Price/Cost Schedule: Quantity is showing 1.00. Should the Quantity be listed as 12 instead of 1.00? Please explain. If so, will an amendment be provided? The service says they will average about 48 patients a year, that is why its lumped into quantity of 1.

2. Per the Price/Cost Schedule: Unit is showing JB. Should the Unit be listed as Months? Please explain what JB stands for? If so, will an amendment be provided? JB means JOB. The unit of measure JB is sufficient for this service. There may be a month you might not provide services

3. 1.6.17 - Combined Synopsis/Solicitation Notice - Other Direct Costs - Please explain if cost needs to be included in the Price/Cost Schedule Total amount? There is only the one line item per year, so provide the cost that it will take to do the service per year. Not sure if this answers your question

4. Please confirm what section of the proposal should include the statement regarding the Terms and Conditions. There is no designated section.

5. Can Contractor personnel work remotely for the new Solicitation? If so, will the VA be providing laptops and how many? Yes, a Contractor can work remotely. There is no VA office space available will be doing work mostly at patient beside. A VA laptop, access to CPRS , access to our shared drive where we put in referrals, etc. You will use an ROI when first reaching out to Veterans and their families as well.

6. Per the new Solicitation - 1.6.6 - Security Requirements: It states, Contractor personnel performing work under this contract must have at time of proposal submission and must maintain the level of security required for the life of the contract. Please explain the type of security required for the Contractor personnel ( Account Manager/Technicians) performing work under the contract? Yes, they will go through fingerprinting, PIV card, laptop, etc. This will be time consuming to get through and possibly take several months before you may get started due to all the set-up processes.

7. If the Contractor personnel can work remotely, the Place of Performance for meeting with Veterans and family members to obtain records and/or information is the Ralph H. Johnson VA Medical Center at 109 Bee Street, Charleston, SC 29401, correct? Yes, you will meet with the patient bedside. At the hospital

8. 1.6.14 Key Personnel; It states: Last paragraph, last sentence - Qualifications for all key personnel are listed below: The Solicitation does not list Qualifications. Please provide.

This pertains to the contract manager who will be responsible for managing the contracts performance once started.

9. For the subject solicitation is the VA's expectation that the contractor provides an on-site resource M- F 8am-5pm in the Charleston VA building? Or would the government prefer that the contractor have off-site resources in the Charleston area, available to respond to patient needs.
