About this file
This document is a Summary of Benefits and Coverage for three healthcare plans offered by an employer. The plans are the Choice Plus Plan 1, the Choice Plan, and the HSA Choice Plus Plan. The plans cover a range of medical services including primary care visits, specialist visits, preventive care, diagnostic tests, imaging, prescription drugs, outpatient surgery, emergency services, hospital stays, mental health/substance abuse services, maternity care, and rehabilitation services. Key details include deductibles, copays, coinsurance rates, and out-of-pocket limits for in-network and out-of-network providers. The plans have various limitations and exclusions, such as no coverage for acupuncture, cosmetic surgery, long-term care, routine eye exams, and routine foot care.
The related state and local contract opportunity is an RFP issued by Hays County, Texas seeking an employee benefits financial consultant to advise the County on their self-funded and third-party health and benefits plans. The consultant will represent the County as needed with third-party providers. The RFP has a question due date of 1/4/24 at 5:00 PM and involves the jurisdictions of Hays County and the City of San Marcos.

Summary of Benefits and Coverage: What this Plan Covers & What You Pay For Covered Services
Choice Plus Plan 1
Coverage Period: 01/01/2024 – 12/31/2024
Coverage for: Family | Plan Type: PS1
The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately.
This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, call 1-866-633-2446.or visit welcometouhc.com. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms see the Glossary. You can view the Glossary at www.healthcare.gov/sbc-glossary/ or call 1-866-487-2365 to request a copy.
Important Questions Answers Why This Matters:
What is the overall deductible? Network: $500 Individual / $1,000 Family
Out-of-Network: $5,000 Individual / $10,000 Family Per calendar year.
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Are there services covered before you meet your deductible? Yes. Preventive care and categories with a copay are covered before you meet your deductible.
This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost-sharing and before you meet your deductible. See a list of covered preventive services at www.healthcare.gov/coverage/preventive-care-benefits/.
Are there other deductibles for specific services?
Yes. $5,000 deductible for all morbid obesity services.
This plan has a separate deductible for covered member that must be met before this plan will pay for qualifying service.
What is the out-of-pocket limit for this plan?
Network Ded/Coins OOP limit: $3,500 Individual / $7,000 Family Out-of-Network Medical OOP limit: Unlimited Network Plan Maximum OOP limit: $6,350 Individual / $12,700 Family Out-of-Network Plan Maximum OOP limit:
Unlimited
The Medical out-of-pocket limit is the most you could pay in a year for covered medical services (includes 20% coinsurance, up to $3,000 plus $500 deductible).
The Plan Maximum OOP limit includes Medical OOP, Deductible and copays. If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met.
What is not included in the out-of-pocket limit?
Premiums, balance-billing charges, health care this plan doesn’t cover and penalties for failure to obtain preauthorization for services.
Even though you pay these expenses, they don’t count toward the out-of-pocket limit.

Will you pay less if you use a network provider?
Yes. See myuhc.com or call 1-866-633-2446 for a list of network providers.
You pay the least if you use a provider in the Designated Network. You pay more if you use a provider in the Network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Do you need a referral to see a specialist?
No. You can see the specialist you choose without a referral.
All copayment and coinsurance costs shown in this chart are after your deductible has been met, if a deductible applies.

HSA Choice Plus Plan
Coverage for: Family | Plan Type: PS1
The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately.
This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, call 1-866-314-0335.or visit welcometouhc.com. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms see the Glossary. You can view the Glossary at www.healthcare.gov/sbc-glossary/ or call 1-866-487-2365 to request a copy.
deductible? Network: $3,200 Individual / $6,000 Family
Out-of-Network: $9,000 Individual / $18,000 Family Per calendar year.
Yes. Preventive care is covered before you meet your deductible.
No. You don’t have to meet deductibles for specific services.
What is the out-of-pocket limit for this plan?
Network: $4,000 Individual / $8,000 Family Out-of-Network: $12,000 Individual / $24,000 Family Per calendar year.
The out-of-pocket limit is the most you could pay in a year for covered services.
This plan uses a provider network. You will pay less if you use a provider in the plan's network.
