About this file
This is a Texas Family Code Schedule certification document required for Solicitation No. 601340000066857 related to a Fabric Pre-Engineered Building procurement for the Texas Department of Transportation (TxDOT) Lubbock district. The schedule serves as a compliance certification document that respondents must complete, sign, and return with their bid submissions. Respondents are required to certify their eligibility to receive state contract payments under Texas Family Code Section 231.006, which addresses child support obligation compliance. Any response must include the names and Social Security numbers of all individuals or business entities holding at least twenty-five percent ownership interest in the submitting business entity. Failure to sign and return this completed schedule may result in the response being deemed non-responsive and subject to rejection.
The document emphasizes that respondents acknowledge potential contract termination and payment withholding if the certification is inaccurate or incomplete. Under the Federal Privacy Act, disclosure of Social Security numbers is mandatory pursuant to Texas Family Code Sections 231.006(c) and 231.302(c)(2), with the information used solely to identify persons owing child support obligations. The state commits to maintaining confidentiality of disclosed Social Security numbers to the fullest extent permitted by Section 231.302(e) of the Texas Family Code. Respondents not meeting the twenty-five percent ownership criteria may check a box indicating the schedule is not applicable and proceed accordingly.
