About this file

This is an Application for Texas Identification Number (AP-152 Form) issued by the Texas Comptroller of Public Accounts, required for entities seeking to bill state government agencies for goods, services, refunds, or public assistance. The form serves as a prerequisite document for state contract opportunities in Texas and must be completed by all persons, sole proprietors, partnerships, corporations, or organizations intending to conduct business with state agencies. The form captures essential payee information including identification numbers, mailing addresses, ownership structure, and payment assignment details necessary for processing billings and maintaining vendor records within the Texas Identification Number System (TINS).

The application requires submission of either an Employer Identification Number (EIN), Social Security Number (SSN), Individual Taxpayer Identification Number (ITIN), or a Comptroller-assigned number, depending on the entity type and ownership structure designated in Section 3. Payees must disclose current Texas tax reporting status and indicate whether the account is new or existing, with the option to establish additional mail codes as needed. The form permits payment assignment to another party when accompanied by a copy of the assignment agreement. Completed applications should be returned to the state agency with which the applicant is conducting business to expedite processing; signatures from the payee are not required if a state agency representative completes the form on their behalf. For assistance, applicants may contact the Texas Comptroller's office at 1-800-531-5441, extension 6-8138.
