About this file

This is a Summary of Benefits and Coverage (SBC) document for the Seton Open Access Aetna Select ACO health plan offered by Travis County Healthcare District - Central Health, effective for the coverage period January 1, 2026 through December 31, 2026. The plan is an Exclusive Provider Organization (EPO) offering coverage for individuals and families through the Seton Health in-network provider network. Key coverage features include an individual deductible of $1,000 and family deductible of $2,000, with an out-of-pocket limit of $4,000 for individuals and $8,000 for families. In-network primary care visits carry a $15 copay, specialist visits are $30, and preventive care is covered at no charge before the deductible is met. Emergency room care requires a $500 copay plus 20 percent coinsurance, while urgent care visits are $50. Prescription drug coverage includes generic drugs at $15 for 31-day supplies and $37.50 for 32-90 day supplies, preferred brand drugs at $40 for 31-day supplies and $100 for extended supplies, and non-preferred brand drugs at $75 for 31-day supplies and $187.50 for extended supplies. Out-of-network providers are generally not covered except for emergency services, which are paid at the same rate as in-network care.

The plan covers various services including diagnostic tests at no charge, imaging services at 20 percent coinsurance, mental health and substance abuse treatment with office visits at $30 copay, and maternity care with professional and facility services at 20 percent coinsurance. Rehabilitation services are limited to 20 visits per year at $30 per visit, home health care is limited to 60 visits per calendar year at 20 percent coinsurance, and skilled nursing care is capped at 60 days per calendar year. The plan excludes bariatric surgery, cosmetic surgery, adult dental care, glasses, long-term care, and non-emergency care outside the United States. The document includes coverage examples demonstrating out-of-pocket costs for maternity care (total patient cost of $2,870 on a $12,700 example event), diabetes management (total patient cost of $1,220 on a $5,600 example event), and emergency fracture care (total patient cost of $1,400 on a $2,800 example event). The plan meets Minimum Essential Coverage and Minimum Value Standards. Aetna administers the plan and provides customer service at 1-888-982-3862, with additional support available through multiple language assistance services at no cost.

Summary of Benefits and Coverage: What this Plan Covers & What You Pay for Covered Services TRAVIS COUNTY HEALTHCARE DISTRICT - CENTRAL HEALTH : Seton Open Access® Aetna SelectSM - ACO

Coverage Period: 01/01/2026-12/31/2026

Coverage for: Individual + Family | Plan Type: EPO

The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately. This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, www.HealthReformPlanSBC.com or by calling 1- 888-982-3862. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at https://www.healthcare.gov/sbc-glossary/ or call 1-888-982-3862 to request a copy.

Important Questions Answers Why This Matters:

What is the overall deductible?

Seton Health In-Network: Individual $1,000 / Family $2,000.

Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.

Are there services covered before you meet your deductible?

Yes. In-network office visits, prescription drugs & preventive care are covered before you meet your deductible.

This plan covers some items and services even if you haven't yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible.

See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits/

Are there other deductibles for specific services? No. You don’t have to meet deductibles for specific services.

What is the out-of-pocket limit for this plan?

Seton Health In-Network: Individual $4,000 / Family $8,000.

The out–of–pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out–of–pocket limits until the overall family out–of–pocket limit has been met.

What is not included in the out-of-pocket limit?

Premiums, balance-billing charges & health care this plan doesn't cover. Even though you pay these expenses, they don’t count toward the out–of–pocket limit.

Will you pay less if you use a network provider?

Yes. See www.aetna.com/docfind or call 1-888- 982-3862 for a list of Seton Health In-Network providers.

This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider's charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.

Do you need a referral to see a specialist? No. You can see the specialist you choose without a referral.
