About this file

This is a Child Support Certification form issued by the Texas Workforce Commission's Vocational Rehabilitation Services division in compliance with Texas Family Code Section 231.006. The form serves as a mandatory certification requirement for all bids, applications, and contracts submitted to the state of Texas for grants, loans, or contract payments. The document requires vendors and applicants to provide the names and Social Security numbers of all individuals with ownership interests of at least 25 percent in the business entity, along with sole proprietors and partners. Contractors must certify under penalty of contract termination that neither the individual nor the business entity is ineligible to receive state funds due to child support delinquencies exceeding 30 days.

The certification establishes that individuals or entities delinquent in child support payments remain ineligible to receive state contract payments until all arrearages are satisfied, a written repayment agreement or court order is in compliance, or the court of continuing jurisdiction grants an exemption as part of a court-supervised effort to improve earnings. The form stipulates that any false or inaccurate certification may result in contract termination and payment withholding, with the vendor liable to the state for attorney's fees, costs necessary to complete the contract including re-advertising and awarding a second contract, and any other damages provided by law or contract. The authorized contractor representative must sign and date the certification attesting to the accuracy of the submitted information.
