About this file

This is a Vendor Reference Form issued by the Texas Cyber Command (TXCC) in support of Request for Offer (RFO) 371-00014 for Texas Networks Operational Maneuver capabilities. The form serves as a structured evaluation tool requiring vendors to provide references from past customers who have purchased similar products or services within the preceding two years. Vendors must submit completed reference forms from their customers directly to TXCC at txcc.procurement@txcc.texas.gov no later than May 21, 2026 at 3:00 PM CST. The form collects comprehensive performance data across multiple dimensions including the vendor's ability to deliver products or services timely, technical knowledge, problem resolution capability, invoice accuracy and timeliness, adherence to delivery schedules, communication effectiveness, personnel professionalism, and overall customer satisfaction. References are scored on a four-point scale ranging from Unsatisfactory (0) to Exceeds Expectations (3), with provisions for Not Applicable ratings where necessary.

The reference evaluation criteria provide explicit scoring definitions to ensure consistency in assessments. An Excellent rating (3) indicates no quality problems, cost issues, or delays with effective and responsive issue resolution. A Satisfactory rating (2) indicates nonconformances, cost issues, or delays that do not compromise contract requirement achievement, with usually effective responses. A Marginal rating (1) denotes issues requiring minor agency resources to ensure compliance, while an Unsatisfactory rating (0) indicates nonconformances, delays, or cost issues that compromise achievement of contract requirements. References must also provide yes or no responses regarding whether the vendor should be used again or recommended to others, and raters must include their name, date, organization, title, phone number, and email address for validation purposes.
