About this file

This is a Proposal Cover Sheet for RFP #126SD100-6390 issued by the State of Texas for Construction Monitoring and Related Services for Marine Mammals for the Bolivar Ferry Project. The document serves as the formal submission template that proposing firms, institutions, or individuals must complete and submit as part of their bid response. The cover sheet requires proposers to provide organizational information, contact details, and identification of the proposer's supervisor who will manage the contract work if awarded. Proposers must also disclose any subproviders involved in the project, including the specific services to be provided and the percentage of work each subprovider will perform. The document establishes that the identified supervisor must be currently employed by the proposing firm and cannot be replaced without prior written consent from the State.

By signing the Proposal Cover Sheet, the proposer's representative certifies that qualified staff are available to fill all positions specified in the RFP, confirms agreement with the teaming arrangement of all subproviders, acknowledges acceptance of the specified rate structure, and agrees to the proposed contract terms. The representative must also certify that the proposer, all staff, and all subproviders have no financial, personal, or business-related conflicts of interest in conducting scientific services for the State of Texas. This certification is fundamental to the proposal's validity and the integrity of the selection process.
