About this file
This document is a Texas Family Code Schedule for Solicitation No. 601340000061515, related to child support obligations and state contract eligibility requirements. The schedule requires respondents to provide information about ownership interests and social security numbers for individuals holding at least 25% ownership in the business entity submitting a response. The document outlines specific requirements for potential contractors to verify their eligibility to receive state grants, loans, or contract payments.
The schedule includes a Federal Privacy Act notice explaining that social security number disclosure is mandatory under Texas Family Code sections 231.006(c) and 231.302(c)(2), with the SSN to be used for identifying persons who may owe child support. Respondents must either complete the ownership information section or check a box indicating the schedule is not applicable. The document emphasizes that failure to sign and return this page may result in the response being considered non-responsive, and warns that the contract may be terminated and payment withheld if the certification is found to be inaccurate.
