About this file
The document is a Texas Family Code Schedule for Solicitation No. 601340000061589, relating to ineligibility to receive state grants, loans, or contract payments based on child support obligations. The schedule requires respondents with at least 25% ownership in a business entity to disclose names and social security numbers of owners. Respondents must certify their eligibility to receive state payments, with an acknowledgment that the contract may be terminated if the certification is inaccurate. The document provides a mechanism for respondents to declare the schedule as not applicable if they do not meet the specified ownership criteria.
The schedule is part of a legal compliance process that ensures vendors have met child support obligations before receiving state contract payments. It includes a Federal Privacy Act notice explaining that social security number disclosure is mandatory under Texas Family Code sections 231.006(c) and 231.302(c)(2), with assurances that the information will be kept confidential. The document must be signed and returned with the response, and failure to do so may result in the response being considered non-responsive. The form was revised on September 27, 2023, and is specifically associated with a solicitation for Commercial Pest Control Services for the Abilene District of the Texas Department of Transportation (TxDOT).
