About this file

This is a Cost Proposal template (Exhibit K) for the Texas Health Care Information Collection Health Care Data Collection System (RFO No. HHS0014117). The contract includes a 3-month transition period, followed by a 9-month Year 1, and full 12-month terms for Years 2-3 (initial term), plus two optional 12-month extension periods (Years 4-5), for a total potential contract duration of 60 months. The transition phase requires multiple deliverables including project management plans, security assessments, and training plans due within 14-90 calendar days after the contract effective date. The operational phase spans the remainder of the contract term, with turnover activities required 180 days prior to contract expiration.

The pricing structure is divided into three components: Transition, Operations, and Turnover costs. All pricing must be all-inclusive, covering labor, materials, licenses, maintenance, tools, supplies, equipment, personnel, and travel expenses. Operations costs are structured as monthly fees, while transition and turnover components are fixed-fee deliverables. The evaluation will use a formula where the lowest total cost proposal receives the highest rating: [1-(Total Cost - Lowest Total Cost)/Highest Total Cost] x Max Percentage Available for Cost Criterion. Respondents must submit their cost proposals in Excel format without modifying locked cells or the structure of the workbook.
