About this file
This is a Fork Maintenance Inspection Form based on NAVSUP Publication 538, Figure 8-13, used for documenting the condition and serviceability of material handling equipment (MHE) forks in U.S. Navy supply operations.
The form requires inspection and documentation of each fork at intervals not exceeding 12 months or whenever defects or permanent deformation are detected. Any identified defect mandates rejection and removal of the MHE from service until repair or replacement occurs. The inspection evaluates both left and right forks across nine inspection criteria: manufacturer's Safe Working Load (SWL) markings, surface cracks, straightness of blade and shank (upper and front faces), fork angle deviation from original specification, difference in height of fork tips, blade thickness measurements with allowable wear tolerances (0.10 inches maximum), positioning locks/latches/stops, fork hooks, and plating condition for Type EX forks. Each fork receives a Satisfactory (SAT) or Unsatisfactory (UNSAT) rating, with overall condition determination based on the cumulative inspection results. The form captures equipment identification (USN, manufacturer, model, year, serial number, contract number), inspector credentials, inspection date, and next due date for follow-up inspection.
