About this file
This is an Emergency Purchase Statement issued by the State of Illinois Chief Procurement Office for General Services on behalf of the Illinois Department of Public Health (IDPH), Disaster Planning and Readiness Division. The document authorizes an emergency procurement for continued maintenance and repair services of 365 state-owned ventilator units maintained in the Strategic National Stockpile. The procurement covers inspection, preventive maintenance, testing, repair, coordination and installation of replacement parts, performance verification, and logistical support necessary to maintain all ventilators in fully operational condition in accordance with manufacturer specifications. The emergency contract is limited to a 90-day period, with an expected start date of February 8, 2026, and an estimated end date of May 8, 2026. This temporary agreement is intended to maintain service continuity while IDPH completes a competitive bidding process for a long-term contract (IFB 25-482DPH-FINAN-B-48312).
The vendor selected is Agiliti Health, Inc., located in Springfield, Illinois, at 422 South 5th Street, with a contract value of $650,973.60 in actual costs. The procurement is justified as necessary repairs to state property to prevent further loss or damage, and the incumbent vendor is uniquely qualified due to its established expertise in maintaining the specific ventilator inventory, completion of prior inspections and repairs, and maintenance of detailed service documentation for each unit. Transitioning to a new vendor would require reinspection of all units at an estimated cost exceeding $95,000 and pose unacceptable delays to public health preparedness and federal funding. IDPH conducted best efforts to include Business Enterprise Program (BEP) certified vendors by searching 79 vendors under NIGP code 938-78, identifying only four BEP-certified vendors, two of whom rely on subcontractors and two of whom could not be reached. The contract is funded through federal funds and was authorized by Mike Caceres, Agency Procurement Office, on February 11, 2026.

Emergency Category (select the appropriate option from the drop-down menu below):
Immediate expenditure is necessary for repairs to State property to protect against further loss or damage
Reason for selection of this particular vendor:
A fourth emergency procurement is necessary to ensure continued maintenance and repair of the State ventilator cache following the expiration of the prior emergency contracts.
Agiliti Health Inc. is uniquely qualified to provide these services on an emergency basis due to its established expertise and ongoing maintenance of the three hundred sixty five ventilators currently in State inventory. The vendor has provided uninterrupted service under the original one year agreement and subsequent emergency procurements, has completed inspections, initiated repairs, and maintains detailed service documentation for each ventilator.
Transitioning to a new vendor during this emergency period would require full reinspection of all ventilators, resulting in estimated costs exceeding ninety five thousand dollars and significant delays in operational readiness. Such delays would pose unacceptable risk to public health preparedness and could jeopardize federal funding.
This fourth emergency procurement is limited to a ninety day period and is intended solely to maintain continuity of critical services while the Illinois Department of Public Health completes and awards a competitively bid solicitation, IFB 25-482DPH- FINAN-B-48312, for a long term agreement.
Description of supplies or services to be provided:
This fourth emergency procurement provides for the continued inspection, preventive maintenance, testing, repair, and documentation of three hundred sixty five State owned ventilator units in accordance with manufacturer specifications. Services include coordination and installation of replacement parts, performance verification, and logistical support necessary to return all ventilators to a fully operational, ready for use condition.
Ventilators shall be returned to the Strategic National Stockpile Regional Storage Site warehouse or shipped directly to healthcare facilities as required to support emergency response operations.
These ventilators were originally procured by the Illinois Emergency Management Agency and transferred to the Illinois Department of Public Health under an intergovernmental agreement. IDPH is responsible for ensuring the ongoing maintenance, readiness, and availability of these assets to support statewide public health emergency response.
This ninety day fourth emergency agreement ensures continuity of essential services while the agency transitions to a new competitively procured contract.

Emergency procurements shall be made with as much competition as is practicable under the circumstances, and agencies shall utilize best efforts to include contractors certified under the Business Enterprise Program in its emergency procurement process. 30 ILCS 500/20-30(a).
List the actions taken by the agency that constitute its best efforts to include contractors certified under the Business Enterprise Program in this emergency purchase:
In compliance with 30 ILCS 500/20-30(a), the Illinois Department of Public Health (IDPH) made best efforts to include Business Enterprise Program (BEP) certified vendors in its emergency procurement process. A vendor search was conducted using NIGP code 938-78 (Hospital Equipment Maintenance and Repair Services), which identified a total of 79 vendors. Of these, only four vendors were certified under BEP.
Further evaluation revealed that two of the BEP-certified vendors rely on subcontractors to perform the actual work and therefore could not directly fulfill the requirements. The remaining two BEP vendors could not be reached despite multiple outreach attempts.
Due to the extremely limited availability of qualified BEP-certified vendors and the urgency of continuing critical ventilator maintenance services, IDPH determined that proceeding with the incumbent vendor was the only viable option.
For these reasons, the agency concluded that it was not practicable to utilize BEP-certified vendors for this emergency contract.
