About this file

This is an Emergency Purchase Statement filed by the State of Illinois Department of Public Health (IDPH), Office of Health Care Regulation (OHCR), located in Springfield, Illinois. The emergency procurement addresses an immediate need for qualified nursing staff to inspect nine long-term care facilities that have fallen into financial or physical distress. The services involve a team of two trained and certified nursing inspectors who will conduct comprehensive facility assessments, including noting regulatory compliance improvements, verifying continued compliance with regulations, observing care provision, reviewing records, intervening on behalf of residents not receiving necessary care, meeting with residents to discuss their services, advising residents of their entitlements, and determining if facilities are failing to meet direct care needs. The contract has an expected start date of April 22, 2024, and an estimated end date of July 21, 2024.

The emergency purchase is being awarded to Certisurv, LLC, a vendor located in Columbia, Tennessee (PO Box 654, ZIP 38401), based on their prior service to the agency and their ability to respond immediately with trained and certified nursing inspectors. The estimated contract value is $96,750, with funding sourced from Federal funds. The emergency procurement was categorized as involving a threat to public health or public safety. Although no existing state contract was identified as available for these services, IDPH is concurrently developing an Invitation for Bid (IFB) to establish a long-term monitoring contract and intends to engage the Business Enterprise Program (BEP) community in that process. The emergency purchase was authorized by Agency Procurement Officer Mike Caceres on April 26, 2024, and was determined to have been made with as much competition as practicable under the circumstances, with best efforts applied to include BEP-certified contractors.
