About this file
This is an Emergency Purchase Statement issued by the Illinois Department of Public Health (IDPH), Disaster Planning and Readiness Division, in coordination with the State of Illinois Chief Procurement Office for General Services. The emergency procurement authorizes maintenance and repair services for 365 ventilator units stored at the Strategic National Stockpile (SNS) Regional Storage Site warehouse in Bloomington, Illinois. The services include inspection, preventive maintenance, repair coordination, parts procurement, performance verification, and logistical management to ensure all ventilators remain fully operational and ready for deployment to healthcare facilities or return to the warehouse in accordance with manufacturer guidelines. The contract term is 90 days, commencing November 10, 2025, and concluding February 7, 2026, with no renewal options specified. This is the third consecutive emergency procurement following the expiration of the initial one-year agreement and a subsequent 90-day emergency contract, necessitated by the non-approval of an extension request.
The total contract value is $650,973.60 in actual costs, funded entirely through Federal Funds. Agiliti Health, Inc., located at 7 Finance Dr, Units 7-9, Bloomington, Illinois 61704, is the incumbent vendor selected due to its established expertise, uninterrupted service history, and detailed documentation on each ventilator's condition. The agency determined that engaging a new vendor would incur estimated re-inspection costs of $95,000 and create significant delays in maintaining critical public health assets, risking loss of federal funding and compromising emergency response capability. In compliance with 30 ILCS 500/20-30(a), IDPH conducted a vendor search identifying 79 vendors under NIGP code 938-78 (Hospital Equipment Maintenance and Repair Services), of which only four were Business Enterprise Program (BEP) certified. Two BEP-certified vendors relied on subcontractors and could not directly fulfill requirements, while the remaining two could not be reached despite multiple outreach attempts. IDPH concluded that proceeding with the incumbent vendor was the only viable option and that BEP-certified vendors were not practicable for this emergency procurement. A new competitive solicitation (25-482DPH-FINAN-B-48312) for a long-term agreement is being prepared and will be publicly advertised and competitively awarded as soon as possible.

Emergency Category (select the appropriate option from the drop-down menu below):
Immediate expenditure is necessary for repairs to State property to protect against further loss or damage
Reason for selection of this particular vendor:
An extension to the second emergency procurement was not granted approval, so a 3rd emergency notice request is necessary to ensure continued maintenance and repair of the State’s ventilator cache, following the expiration of the initial and secondary emergency contracts. Agiliti Health, Inc. is selected for this short-term emergency due to its established expertise, ongoing service, and unique familiarity with the 365 ventilators currently under care.
Agiliti Health, Inc. has provided uninterrupted service under the original one-year agreement and the recently expired 90-day emergency contract. The vendor has completed full inspections, initiated repairs, and maintains detailed documentation on each ventilator's condition. This deep familiarity ensures efficient continuation of work without the delays or costs associated with on-boarding a new vendor.
Engaging a new vendor would require returning all ventilators for re-inspection, incurring estimated costs of $95,000 and significantly delaying the readiness of these critical public health assets. Such delays risk loss of federal funding and could compromise the State’s ability to respond to public health emergencies.
IDPH is now seeking a third emergency contract to continue these critical services beyond August 10, 2025, while a new competitive contract is finalized and awarded. the Illinois Department of Public Health (IDPH) is preparing to issue a new solicitation, 25-482DPH-FINAN-B-48312 IFB for long term agreement. This new solicitation will be publicly advertised and competitively awarded as soon as possible.
This third emergency contract is limited to a 90-day period to provide continuity of service while the new solicitation process is completed. The pricing has been reviewed and deemed fair and reasonable based on prior negotiations and the vendor’s existing work. Continuing with Agiliti Health, Inc. under this short-term third emergency contract is the only practical solution to maintain ventilator readiness and protect public health.
Description of supplies or services to be provided:
The Illinois Department of Public Health (IDPH) is seeking a second emergency procurement to extend services for the administrative and logistical management of the State’s ventilator cache, which is stored at the Strategic National Stockpile (SNS) Regional Storage Site (RSS) warehouse. This third emergency contract will sustain uninterrupted maintenance and repair services for these critical devices while a new competitively bid contract is finalized and awarded.
The services provided under this contract include the continued inspection, preventive maintenance, and repair of 365 ventilator units in accordance with manufacturer guidelines. This also includes the coordination of parts procurement, Page 2 of 3State of Illinois Chief Procurement Office for General Services Emergency Purchase Statement V.25.2

completion of repairs, performance verification, and logistical management for the return or deployment of the units. All ventilators must be returned to a fully operational, ready-for-use state, either shipped directly to healthcare facilities or returned to the RSS warehouse.
These ventilator devices, originally procured by the Illinois Emergency Management Agency (IEMA), were transferred to IDPH under an Intergovernmental Agreement. IDPH is now responsible for ensuring their ongoing maintenance and operational readiness to support emergency response operations across the state.
This 90-day third emergency agreement ensures critical continuity of services while the agency transitions to a new solicitation.
Emergency procurements shall be made with as much competition as is practicable under the circumstances, and agencies shall utilize best efforts to include contractors certified under the Business Enterprise Program in its emergency procurement process. 30 ILCS 500/20-30(a).
List the actions taken by the agency that constitute its best efforts to include contractors certified under the Business Enterprise Program in this emergency purchase:
In compliance with 30 ILCS 500/20-30(a), the Illinois Department of Public Health (IDPH) made best efforts to include Business Enterprise Program (BEP) certified vendors in its emergency procurement process. A vendor search was conducted using NIGP code 938-78 (Hospital Equipment Maintenance and Repair Services), which identified a total of 79 vendors. Of these, only four vendors were certified under BEP.
Further evaluation revealed that two of the BEP-certified vendors rely on subcontractors to perform the actual work and therefore could not directly fulfill the requirements. The remaining two BEP vendors could not be reached despite multiple outreach attempts.
Due to the extremely limited availability of qualified BEP-certified vendors and the urgency of continuing critical ventilator maintenance services, IDPH determined that proceeding with the incumbent vendor was the only viable option.
For these reasons, the agency concluded that it was not practicable to utilize BEP-certified vendors for this emergency contract.
