Scope of Work for Elevator, Lift, and Dumbwaiter Inspections
A.1 GENERAL INTENT:
Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to furnish elevator inspection services on equipment (list provided separately) located at the VA Medical Center, 500 West Fort Street, Boise, ID in accordance with the terms, conditions, provisions, specifications, and schedules of this proposal.
A.2 SIGNING IN AND OUT:
a. The following offices are the required contact offices for the Contractor at the time indicated:
Monday – Friday 8:00am – 4:30 pm Facility Management Service - Bldg. 50, room, 101 Phone #: 208-422-1370
During hours when the above office is closed the Contractor shall report to:
Admitting Office - Bldg. 85, room. 112A Phone #: (208) 422-1109.
b. For each visit, the Contractor shall report in person to the appropriate office, both upon arrival and prior to departure, to sign “in” and “out” on response log.
A.3 INCLUDED WORK
a. The Boise Veterans Administration Medical Center -VAMC- requires that all elevators and dumbwaiters be inspected by an independent and certified elevator inspector. The consultant must carry a current Qualified Elevator Inspector’s (QEI) certification. All inspections will be in accordance with ASME A17.1, ASME A17.2, and ASME A17.3.
The consultant will coordinate inspections with the COR, and with the elevator maintenance contractor. The inspections will be completed during normal business hours.
A.4 SERVICE VERIFICATION AND SERVICE REPORT
a. Contractor shall observe check-in/out procedures described above, so the VA will be aware of the Contractor’s presence at the facility at all times.
Upon each departure, Contractor shall serve notice of disposition of work to the Facility Management contact office to include:
1. Contract purchase order number.
2. Date service initiated.
3. Date completed.
b. VA shall be allowed the opportunity to have designated VA employee or competent contracted representative observe all work performed in fulfillment of the contract.
A.5 COORDINATION WITH EQUIPMENT USERS
a. Once on site, Contractor representatives are required to coordinate performance of work with Facility Management Service. Depending upon the situation, the Contractor may be directed to notify an “area supervisor” of their presence as well as of possible disruptions that may affect their operations. At all times, the contractor representative shall respond to inquiries from VA personnel regarding any aspect of the work they are performing or shall direct them to the Facility Management Service. If requested, the Contractor may be required to notify area supervisor prior to leaving an area for the day. Small occasional delays (up to one hour per occasion) in the contractor’s progress due to coordination issues with the equipment users are considered a normal part of work; but the Boise VA will make every effort to eliminate hold-ups to work progress.
A.6 DOWNTIME LIMITATIONS
a. Contractor shall provide service which limits both “full downtime” and “partial downtime”. Full downtime is defined as loss of essential equipment functionality, while partial downtime refers to all other actions that will limit the operation of the elevators, lifts, and dumbwaiters
A.7 SAFETY
The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractors’ fault or negligence.
A.8 PARKING/TRAFFIC RULES:
Parking is available for contractor vehicles on the VA grounds. It is the responsibility of the contractor to park in the appropriate designated parking areas and to observe all traffic regulations while on VA facilities. The VA will not invalidate or make reimbursement for parking or traffic violations of the contractor
