About this file

This document is the Attachment B Price Sheet for ITB No. 24-84131512-ITB Electronic Data Processing Equipment Insurance, issued by the State of Florida, Department of Management Services. The Price Sheet provides instructions for brokers to complete the price quote for the insurance coverage required, which includes owned and leased/rented/borrowed electronic data processing equipment, medical and scientific equipment, and UF Scripps scientific equipment. Brokers are required to provide a rate per $100 of total insured value for each line item and identify any applicable assessments, fees, or taxes. The Price Sheet automatically calculates the premium subtotal, total premium, discounted premium, and calculated premium for the four-year policy period. Responses must be submitted via the Ariba on Demand Sourcing event in MyFloridaMarketPlace.

The pricing instructions outline the formulas used to calculate the discounted and calculated premiums, as well as requirements regarding alterations to the Price Sheet and the inclusion of fees. Brokers are not allowed to include terrorism risk insurance premiums, as this coverage is not required. The selected broker will not be permitted to impose any fees not listed in the Price Sheet, unless required by statute.
