About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Veteran Affairs (DVA) for the installation of 15 Elkay model LZWSM8K or equivalent surface mount bottle filling stations at the DVA Manteno facility. The project requires the successful vendor to supply and install the bottle filling stations, relocate an existing ice machine, and perform associated carpentry, plumbing, and electrical work. The RFQ specifies the quantity, location, and technical specifications for each bottle filling station, as well as the scope of work for the related tasks. Quotes must be submitted by the due date listed in the BidBuy system.
The RFQ outlines the pricing and commercial terms, including the requirement for a minimum 1-year warranty on all products. It also states that the selected vendor must be able to provide all required supplies and services and offer the lowest price. The RFQ notes that state agencies shall only accept electronic submissions for small purchase procurements via email or BidBuy, and that manual submissions in paper format will not be accepted.
