All items listed in the ship Survey Report Index, which apply to the ship, are to be described in sufficient detail to furnish a comprehensive picture of existing conditions. Such descriptions are to be listed on separate sheets and identified by the Index number. Where applicable, note the means used determine the condition of an item, e.g., hammer testing, audio-gauging, drilling, megger testing, etc.
In addition to the description of the structural condition of an item, indicate the following data: manufacturer, number, type, capacity, prime mover, controls, location, and coatings. Identify all shell plating by number and letter start-ing from stem and flat keel respectively.
Place each item in one of the following categories and note by number (1), (2), (3), (4) or (5) in the right hand margin of the report:
(1) Satisfactory—Good to excellent; condition superior in all respects or better than average; no maintenance or repair required.
(2) Record—Fair to good; condition average, deficiencies of a minor nature not requiring correction.
(3) Deferred—Poor to fair; condition below average; deficiencies requiring immediate correction or those carrying a grace period granted by regulatory bodies for performance thereof.
(4) Unexamined—Conditions which could not be determined due to ship being afloat, cargo laden, etc.
(5) Unsighted Internally—Conditions which could not be determined due to equipment, tanks, etc., not opened for internal examination; exterior condition of such items to be commented on.
