Control Road, Houston Mesa Road and Reynolds Creek Bridges

Place the SWPPP and all updates in a three-ring binder so that completed inspection forms and other records may be inserted. Make the SWPPP available for public inspection and for use by the CO.

Revisions to the SWPPP may be necessary during construction to make improvements or to respond to unforeseen conditions noted during construction or site inspections. For that purpose, specify in the SWPPP the mechanism whereby revisions may be proposed by the Contractor or the CO and incorporated into the plan, including review and approval of minor changes. Jointly approve and sign each revision to the SWPPP. Implement approved changes according to the

CGP.

(e) Notice of Termination (NOT). File a NOT when the conditions listed in the CGP have been met or transfer the NOI to FHWA when project has reached final acceptance. The Government will file a NOT.

At the completion of the project, provide the CO with the complete SWPPP, including inspection forms, logs, and all other required documentation added during the project.

107.02 Protection and Restoration of Property and Landscape. Delete the sixth paragraph and substitute the following:

Before beginning work in an area, contact the local Utility Locator Service, at the phone number shown in the plans, to have all utilities located. Protect utilities from construction operations.

Cooperate with utility owners to expedite the relocation or adjustment of their utilities to minimize interruption of service, duplication of work, and delays.

Add the following:

Noxious weed prevention practices, as provided to the CO by the Tonto National Forest in Section 625, will be adhered to during project construction.

The locations of the utilities shown in the plans have been certified to a Quality Level C, with spot locations certified to a Quality Level C according to the CLFHD Utility Data Quality Certification requirements:

Section 625. - TURF ESTABLISHMENT

Description

625.01 Add the following:

This work also consists of noxious weed/exotic plant prevention and control in accordance with Noxious Species Control Plan (NSCP) and Pesticide Use Proposal (PUP) prepared by the Contractor.

Construction Requirements

625.013. General. Add the Following:

Perform noxious weed/exotic plant prevention and control prior to any ground disturbing activities in accordance with the approved plan.

All seed mixes to be used on the Tonto National Forest are required to be certified weed free for those seeds listed on the Tonto weed list. This is in addition to the requirement of all seed being pure live seed. The following is the procedure for purchasing seed to be used on the Tonto National Forest.:

Purchase your seed from a licensed seed dealer.

A Certificate of Analysis for each lot of each seed species shall be furnished to the CO at least two weeks prior to seeding activity. No seed shall be furnished to, or delivered to the project until approved by the CO. The Certificates of Analysis shall contain the following information for each seed sample: the test results of the Fifty States Noxious Weed list, all seeds including weed seeds listed, purity and germination, tetrazolium test results, when used and any pathology found to be present. The sample testing shall use the rules for testing seeds published by the “Association of Official Seed Analysts” or the “Society of Commercial Seed Technologists”.

Provide this Certificate of Analysis to the CO, who will forward it to the Forest Noxious Weed Program Manager. Other species on this Certificate will be compared with the Tonto National Forest list of noxious weeds. The list of noxious, restricted or invasive species is located at the following website:

http://fs.usda.gov/goto/tonto/weeds

If the samples indicate species listed as noxious, restricted or invasive, the lot may be rejected for use on the project. The Forest Service will determine whether any lot of seed will be rejected, and will notify the CO within 2 days of receipt of Certificates of Analysis.

The contractor shall provide all seed tag labels to the CO. No payment will be made for seed until tag labels and Certificates of Analysis from all seed to be used on the project have been submitted as specified.

625.09 Noxious Weed/Exotic Plant Prevention and Control and Protecting and Caring for Seeded Areas. Protect and care for seeded areas including watering when needed. Repair or apply supplemental applications of seed, mulch, fertilizer, and water as many times as needed until turf is established or final acceptance.

(a) Noxious weed/exotic plant plan development and approval. Prepare a Noxious Species Control Plan (NSCP) containing the following information:

1) The proposed method of control to be used for each plant species listed below, and at each of stage of plant development.

2) The herbicides, method of application, and rates to be used for each listed plant species.

3) Responsible Applicator and required Structural Pest Control Applicator Licensing information.

4) A record of the dates and locations of such applications or treatments on forms supplied by the CO. A copy of this record shall be submitted to the CO after each application.

5) Methods and procedures to be followed to protect existing, transplanted, and new emerging vegetation in seeded areas.

All materials used shall also be in accordance with the approved Pesticide Use Proposal.

Plants requiring treatment are found in the List of Invasive Species for The Tonto National Forest. This list is available at:

If other plant species listed in the Arizona Noxious Weed List or the Federal Noxious Weed List occur within the project area, the contractor shall modify the Noxious Species Control Plan to add such species, including acceptable control measures and PUP, and re-submit the plan to the CO for acceptance.

The Arizona Noxious Weed List is available electronically at:

The project areas will be surveyed by the C O following rainfall events and during plant germination and growth periods for listed noxious and invasive plant species. When determined to be present within the project right-of-way (project limits) at any time before contract completion the contractor shall treat the areas designated by the CO in accordance with the approved NSCP. Such treatments shall be completed and approved by the CO before ground disturbing or earthmoving activities occur from those areas.

The contractor shall mark those areas receiving manual control with an application of a photosensitive dye.
