About this file

This is a Schedule B Pricing Matrix for the Fisher-McCall 12th Avenue Orphan Well Plugging and Facility Decommissioning Project in Ottawa County, Michigan, administered by the state environmental agency (EGLE). The project encompasses plugging of eight wells and decommissioning of two facilities. The pricing matrix establishes unit rates for all materials, labor, equipment, and services required to complete the well plugging and facility decommissioning work. Line items include contractor supervision, laborers, specialized drilling equipment (pole rigs, derrick rigs, BOPs in multiple sizes), tubing and casing materials in various dimensions, drilling bits, wireline services, cementing services and materials, mud engineering and drilling fluids, waste disposal services, heavy equipment (dozers, backhoes, excavators, cranes), transportation services, site preparation and restoration, infrastructure removal, and flowline services. The bidder must submit pricing for all rated items regardless of anticipated use, provide a project timeline assuming 10-hour workdays, and obtain EGLE Project Manager approval before mobilizing or charging for any items.

All pricing in the matrix currently reflects $0.00 values, indicating this is a blank template awaiting bidder submission. Bidders must provide unit prices and quantities for each line item, with the grand total calculated from these submissions. The contract prohibits double-billing (such as charging both rig crew and laborer rates simultaneously), prohibits invoicing overhead percentages on items already included on the Schedule B, and limits contractor site supervisor charges to one person per day unless approved by the EGLE Project Manager. Any specialized equipment, supplies, services, or materials not listed on the pricing matrix require prior EGLE approval and will be invoiced at cost plus the overhead percentage. Materials necessary for proper well plugging that are not listed on the matrix will also be billed at cost plus overhead.

NOTES:
1. You must submit a project timeline as outlined in DS document. Please assume 10-hour days.
2. Provide Quantity and Price/Unit for all items listed in the Bid Table, even if the item is not likely to be used for the project. We call this 'Rated Items' and have requested the unit pricing in the event
the Scope of Work changes, a change order is necessary, or we move to a time and materials billing.
3. Bidder cannot mobilize, use, or charge for items in the pricing matrix without prior approval from EGLE Project Manager.
4. If a ‘rated item’ is used to complete a ‘lump sum’ item work activity included in the original scope of work, it cannot be invoiced separately.
For example, if you utilize an ‘excavator’ to complete concrete pad removal (‘Other Infrastructure Removal’ bid item), you cannot charge
for the excavator or operators hours as it should have been included as part of the lump sum price for ‘Other Infrastructure Removal’
5. Only 1 Contractor Site Supervisor can be invoiced per day of onsite work, unless authorized and/or approved by EGLE project manager.
6. Rental and/or purchase of any specialized equipment, supplies, services, or materials must have prior approval from EGLE Project Manager.
7. Overhead % cannot be added to the hourly or daily rates of items already included on Schedule B when invoicing.
8. If a material, such as 'casing' is needed to properly plug the well and is not listed on this pricing matrix, it will be invoiced at cost plus the 'Overhead %'
9. Laborer includes but is not limited to: Roustabout, Heavy Equipment Operator, Tool Hand, and other similar positions. The Laborer rate will also be used by the rig crew when they are completing work activities not assoicated
with the rig, including but not limited to: laying mats, moving equipment, loading/offloading equipment and supplies or other similar general labor duties. Rig crew cannot be invoiced at the Laborer and 'Rig Crew'
line items at the same time, as that would be double-billing.
10. You may add additional lines to this pricing matrix at the BOTTOM if needed.
