About this file

This document is a Draft Statement of Work (SOW) for maintenance and improvement of the Nespelem Indian Health Clinic parking areas in Washington State. The project scope includes cleaning and filling parking lot cracks, applying a seal coat to asphalt surfaces, re-striping parking areas with high-visibility paint, adding ADA-compliant markings, and constructing a new sidewalk along San Poil Avenue. Technical requirements specify using commercial-grade asphalt emulsion seal coat, hot rubberized crack sealer, waterborne acrylic traffic marking paint, and ready-mixed concrete, all in compliance with Washington State Department of Transportation standards and various national building codes.

The contractor must meet specific quality assurance requirements, including holding a valid Washington state general contractor's license, OSHA construction safety certification, and providing a two-year warranty. Work execution involves detailed procedures for crack sealing, painting, and concrete pavement installation, with specific guidelines for surface preparation, material application, and testing. The project requires submittal of pre-construction documentation, materials specifications, and post-construction reports, including a completion report, red-lined drawings, and test reports. A kickoff meeting and closeout procedures are mandatory, with the contractor expected to provide an Operation and Maintenance manual and coordinate a final closeout meeting with the Contracting Officer Representative.
