About this file
This is a Draft Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) for chiller preventative maintenance services at Overton Brooks VA Medical Center in Shreveport, Louisiana. The contractor shall provide all labor, materials, tools, equipment, supervision, and transportation necessary to perform preventative maintenance, inspections, testing, and cleaning on three York chillers (Models YKLGLLH9 EVHS with serials SKKM088520, SKKM088530, and SKKM088540) and one Trane chiller (Model CVHE 1N 3B, Serial L03F05451), along with four Evapco cooling towers. The base contract period runs from October 1, 2026 through September 30, 2027, with four optional one-year renewal periods extending through September 30, 2031. Work shall comply with manufacturer requirements, OSHA standards, EPA regulations, and applicable federal, state, and local laws.
The PWS establishes comprehensive maintenance requirements including monthly preventative maintenance tasks (oil system checks, motor testing, voltage balance verification, refrigerant leak testing, and operating condition documentation), annual maintenance activities (electrical tightening, megohm testing, filter and drier replacement, oil and vibration analysis, tube cleaning, and heat exchanger flushing), and cooling tower maintenance (deck and sump cleaning, component inspection, oil changes every five years, and control verification). Contractor qualifications must include EPA 608 Universal Certification, Louisiana HVAC licensure if required, OSHA ten-hour safety training, and lockout-tagout and VA Medical Center safety protocol training. Emergency response is required within four hours for full system outages affecting patient care; urgent response within twenty-four hours for non-critical issues. Deliverables include monthly reports within forty-eight hours, annual reports within five business days, and cooling tower inspection reports within five business days. The QASP establishes 100 percent compliance requirements for monthly and annual maintenance, 95 percent pass rates for cooling tower inspections on first attempt, and on-time reporting targets of 95 percent, with performance evaluated through work inspections, random sampling, customer feedback, and COR oversight. An incentive program offers quarterly bonuses up to three percent and annual excellence awards up to five percent for superior performance, while liquidated damages range from 0.25 percent per day for late monthly reports to one percent per hour for emergency response delays beyond four hours, capped at appropriate maximums per violation category.
