About this file

This is a Past Performance Reference Questionnaire template used to evaluate contractor performance on previous federal contracts. The form consists of two main parts: Part I to be completed by the Offeror with reference information including contractor details, project description, and contract data (contract number, value, period of performance, and role as prime or subcontractor); and Part II to be completed by the reference Point of Contact with performance ratings and narrative feedback.

The questionnaire includes a standardized rating system with five levels: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory. The reference must rate the contractor's performance across eight key areas: quality of services, reliability and timeliness, management effectiveness, business relations, problem resolution, customer satisfaction, small business subcontracting, and overall satisfaction. The form allows for additional narrative justification and requires the respondent's signature. This appears to be a draft template that would be used as part of a larger solicitation package for Vehicle Marshalling and Sales Support Services for GSA Fleet Acquisition Support Branch.

Very Good Performance met contractual requirements and exceeded some to the government’s benefit.

The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory Performance met contractual requirements. The contractual performance of the element or sub-element being evaluated contained some minor problems for which corrective actions taken by the contractor appeared to be or were satisfactory.

Marginal Performance did not meet some contractual requirements.
