Except as otherwise specifically stated in the PO, Vendor shall obtain and maintain insurance as specified in this section at all times during the term of the PO:
A. Workers’ compensation insurance as required by state statute, and employers’ liability insurance covering all Vendor employees acting within the course and scope of their employment;

Commercial general liability insurance written on an Insurance Services Office occurrence form, covering premises operations, fire damage, independent contractors, products and completed operations, blanket contractual liability, personal injury, and advertising liability with minimum limits as follows:
$1,000,000 each occurrence; $1,000,000 general aggregate; $1,000,000 products and completed operations aggregate; and $50,000 any one fire; and

Reference section 21 of the State of Colorado Purchase Order Terms and Conditions for applicable insurance requirements.
2.5 Annual Rate Adjustments
Prices established in the award shall remain firm through June 30, 2027. Beginning with July 1, 2027 and each succeeding July 1 for the term of the award document, rate adjustments may be implemented and will be based on the Consumer Price Index (CPI) https://osc.colorado.gov/spco/central-contracts-unit/purchase-order-terms-conditions rates for All Urban Consumers (Chained). Only one such increase will be allowed in any 12-month period of this Purchase Order. In no event shall the rates set for consecutive fiscal years be less than the prior fiscal year’s prices.
To initiate a rate adjustment, the vendor must formally request the official CPI report from the Program Administrator. Following receipt of the CPI report, the vendor must calculate the new allowable rates and submit a modified bid schedule and a rate adjustment request to the Program Administrator before May 15th of each year for review. No rate adjustment will become valid or enforceable until the State approves a modification of the Purchase Order reflecting the new rates. The new rates will come into effect on July 1st of each year and the new rates will apply to work initiated on or after July 1st of each year.
CPI rate adjustments can be found at the following web address:
http://data.bls.gov – (Select:All Urban Consumers (chained CPI)/All Urban Consumers/ All items/not seasonally adjusted – click the more formatting options and choose 12-month percent change)
Rate adjustments will be calculated using the average of the prior 12 months of Consumer Price Index (CPI) rates for All Urban Consumers (Chained CPI) published prior to the May 15 that the rate increase is due to take effect. The Bureau of Labor Statistics generates CPI. If the All Urban Consumers (Chained CPI) database is discontinued, the State may use an equivalent database at its discretion. To ensure fairness at the time of adjustment, the average percentage will be calculated from the most recent 12-month period for which data is available at a time reasonably in advance of the renewal date. When calculating these rate adjustments, rounding shall be handled as follows: any price increase shall be rounded up to nearest whole dollar if the first decimal place number is 5 or greater; if the first decimal place number is 4 or less the price rate will be rounded down.
