THE PURPOSE OF THIS AMENDMENT IS TO:
1. INCORPORATE THE PAST PERFORMANCE QUESTIONNAIRE WHICH WAS INADVERTENTLY OMITTED FROM THE SOLICITATION.
2. UPDATE SECTION 1.10.2 OF THE STATEMENT OF WORK (SEE CONTINUATION PAGE).
3. NOTIFY PROSPECTIVE 8(a) CONTRACTORS THAT RESPONSES TO QUESTIONS WILL BE POSTED TO FEDBIZOPS BY AMENDMENT ON A WEEKLY BASIS STARTING FRIDAY 3-JUL-2009. LAST DAY FOR POSTED RESPONSES WILL BE 21-JUL-2009.

Indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

E EXCEPTIONAL (BLUE) – The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
V VERY GOOD (PURPLE) – The contractor’s performance meets contractual requirements and exceeds some (requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S SATISFACTORY (GREEN) – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or was satisfactory.
M MARGINAL (YELLOW) – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
U UNSATISFACTORY (RED) – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

Section 1.10.2 of the statement of work has been updated from:
1.10.2 Background Checks/Security: Contractor shall hold, at a minimum, an interim secret facility clearance prior to the RFP closing date. All contractor personnel that require regular access to DOL must submit to the Department’s process for obtaining a Personal Identity Verification (PIV) card. The PIV process includes a background investigation as required by the Homeland Security Presidential Directive-12 (HSPD-12) Policy for a Common Identification Standard for Federal Employees and Contractors,” which the President issued on August 27, 2004.
To the following:
1.10.2 Background Checks/Security: All individuals provided by the contractor shall be a U.S. residents or citizens and must undergo a background investigation. Contractor shall hold, at a minimum, an interim secret facility clearance prior to the RFP closing date. All contractor personnel that require regular access to DOL must submit to the Department’s process for obtaining a Personal Identity Verification (PIV) card. The PIV process includes a background investigation as required by the Homeland Security Presidential Directive-12 (HSPD-12) Policy for a Common Identification Standard for Federal Employees and Contractors,” which the President issued on August 27, 2004, the DOL the Personnel Suitability and Security Handbook, September 2004, the DOL Policy for PIV Card Issuance to New Federal Employees and Contractors, December 2005, and the Federal Information Processing Standards 201.
