About this file

This is an Official Letter of Intent (DMI-40 Form) issued by the City of Tampa's Equal Business Opportunity Program for documenting Small Local Business Enterprise (SLBE) participation in city contracts. The form serves as a mandatory requirement for prime contractors and service providers to formally declare their intent to utilize SLBE subcontractors and suppliers on city bid, proposal, and contract opportunities. The Letter of Intent must be completed and signed by both the prime bidder and the SLBE firm, with submission required within ten (10) workdays after the notice of intent to award is posted. Failure to provide all required letters of intent within the prescribed timeframe may result in bid non-responsiveness or delay in contract award. The form applies to the City of Tampa's Predictive Maintenance Program (Bid No. 25-P-00363) for the David L. Tippin Water Treatment Facility, with a bid opening scheduled for Friday, December 5, 2025, and an estimated contract value of $25,000.00. The initial contract period is one year with potential renewal options for three additional one-year periods, and the City reserves unilateral extension rights for up to three additional 30-day periods.

The DMI-40 Form requires documentation of the agreed-upon cost of work or supplies to be provided by the SLBE and the corresponding percentage of the total city contract amount. Upon execution, a copy of the subcontract agreement and/or purchase order between the prime contractor and SLBE must be filed with the City of Tampa immediately and prior to the commencement of the SLBE's work. Bidders must demonstrate at least five years of vibration analysis services experience, provide customer references, submit testing instrument lists, and document certified technician credentials. The procurement covers eight water department facilities and includes monthly vibration analysis for 138 pieces of equipment, quarterly spectrometric oil analysis, thermographic analysis, and detailed pricing schedules for scheduled testing services, unscheduled field services, and replacement parts. The City reserves the right to award to the lowest responsive, responsible bidder meeting all technical specifications and SLBE participation requirements.
