About this file

The document is a Disqualification Disclosure form associated with the City of Pasadena's Brookside Park Stormwater Capture Project for Engineering Design Services, prepared in accordance with California Public Contract Code §10162. The form requires a prospective bidder to disclose, under penalty of perjury, whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple Yes/No checkbox mechanism for bidders to indicate their disqualification status, with an option to explain circumstances and attach supporting documentation if the answer is "Yes". While the form itself does not contain detailed project information, it is part of the procurement process for the Brookside Park Stormwater Capture Project, which aims to divert and treat stormwater flows, create an 11 acre-feet subsurface infiltration unit, and implement secondary treatment best management practices in the Arroyo Seco subwatershed of the Upper Los Angeles River watershed.
