About this file

This document is a Disqualification Disclosure form prepared in accordance with California Public Contract Code §10162, specifically for the Central Library Retrofit Project Building and Fire Plan Review Services in Pasadena. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple Yes/No checkbox format for bidders to indicate their past project participation status, with an option to explain and attach documentation if the answer is "Yes". While the document does not provide specific pricing, contract terms, or funding details, it is part of the bid qualification process for the Pasadena Central Library's seismic retrofit and repair project, ensuring transparency and accountability in the bidding process for professional plan review services.
