About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162. The form applies to the Contracted Hearing Officer opportunity and requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer of the bidder, or any employee with a proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate either "Yes" or "No" to this question.

If a bidder answers affirmatively, the form requires a detailed explanation of the circumstances along with supporting documentation. This disclosure requirement serves as a qualification screening mechanism to ensure that only vendors without prior disqualification history are eligible to participate in the competitive bidding process for hearing officer services.
