About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for the Managed Detection and Response (MDR) Services procurement (P-67709). The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer of the bidder, or any employee with proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violation of law or safety regulations. Bidders must check either "Yes" or "No" to this question, and if answering affirmatively, must provide a detailed explanation of the circumstances with supporting documentation.

This disclosure requirement is a standard compliance document that does not contain pricing information, set-asides, funding details, or contract terms. Rather, it serves as a qualification screening mechanism to ensure that only eligible contractors participate in the procurement process. The form is part of the broader solicitation package for the City of Pasadena's MDR services contract, which is due December 5, 2025, and carries an initial three-year term with optional two one-year extensions. Submission of this completed disclosure form is a mandatory prerequisite for bid consideration.
