About this file

This document is a Disqualification Disclosure form pursuant to California Public Contract Code §10162, designed for prospective government project bidders. The form requires bidders to disclose whether they, their officers, or employees with proprietary interest have ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.

The form provides a simple binary response option (Yes or No) with an instruction to explain circumstances and attach supporting documentation if the answer is affirmative. The document emphasizes that the disclosure must be completed under penalty of perjury, indicating the serious legal implications of providing false information in this governmental contracting process.
