About this file

This document is a Disqualification Disclosure form for the City of Pasadena's Executive Recruitment Services contract opportunity, prepared in accordance with California Public Contract Code §10162. The form requires prospective bidders to disclose whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple yes/no questionnaire where bidders must indicate if they have any past disqualifications, with an option to explain circumstances and attach supporting documentation if the answer is affirmative. This disclosure is part of the City of Pasadena's procurement process for selecting one or more executive search firms to provide comprehensive recruitment services for executive and senior management positions, with potential for hourly-based contracted work such as reference checking and executive exit interviews.
