About this file

The document is a Disqualification Disclosure form required by California Public Contract Code (PCC §10162), specifically for the Rose Bowl Operating Company's (RBOC) Graphic Designer Request for Proposals. This mandatory questionnaire requires prospective bidders to disclose whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple yes/no checkbox for bidders to indicate their compliance history, with an option to explain and attach documentation if the answer is "yes". This disclosure is part of the procurement process for a one-year graphic design services contract for the Rose Bowl Stadium and Brookside Golf Courses, with potential two one-year renewal options at the discretion of the RBOC Chief Executive Officer / General Manager. The contract will commence on July 1, 2023, and is designed to secure graphic design services for this iconic California venue that hosts UCLA football games, the annual Rose Bowl Game, international soccer matches, concerts, and multi-day music festivals.
