About this file

The document is a Disqualification Disclosure form associated with a California state and local contract opportunity for the Rose Bowl Operating Company (RBOC) and the City of Pasadena. The form, aligned with California Public Contract Code §10162, requires prospective bidders to disclose whether they, their officers, or employees with proprietary interest have ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The disclosure form is a standard compliance document that requires bidders to answer "Yes" or "No" to the disqualification question, with an option to explain circumstances and attach supporting documentation if the answer is affirmative. This form is part of the bid submission process for the engineering services contract for plumbing infrastructure replacement at the historic Rose Bowl Stadium, which aims to ensure contractor integrity and transparency in the public procurement process.
