About this file

This document is a Certification of Non-Involvement in Prohibited Activities form issued by the State of New Jersey, specifically related to N.J.S.A. 52:32-60.1, effective January 22, 2024. The certification requires any vendor seeking to enter into or renew a contract with a state agency to verify their status on the Office of Foreign Assets Control (OFAC) Specially Designated Nationals and Blocked Persons list, particularly regarding activities related to Russia and/or Belarus.

The form provides three certification options: (A) confirming the vendor is not identified on the OFAC list for Russia/Belarus-related activities, (B) acknowledging the vendor is identified on the list due to Russia/Belarus activities, or (C) indicating identification on the list but asserting the activities are consistent with federal law. Vendors must complete the form, providing company details including name, address, contact information, and Federal Employer Identification Number (FEIN). The document outlines potential consequences for false certification, including sanctions, damage recovery, default declaration, and potential debarment or suspension by the Department of the Treasury.
