About this file

The document is a Certification of Non-Involvement in Prohibited Activities form issued by the State of New Jersey, specifically addressing vendor interactions with Russia and Belarus. The certification is pursuant to New Jersey Statute N.J.S.A. 52:32-60.1 and requires vendors seeking to enter into or renew contracts with state agencies to verify their status on the Office of Foreign Assets Control (OFAC) Specially Designated Nationals and Blocked Persons list, specifically regarding activities related to Russia and/or Belarus.

The form provides three certification options: (A) confirming the vendor is not identified on the OFAC list for Russia/Belarus-related activities, (B) acknowledging identification on the list due to Russia/Belarus activities, or (C) being identified on the list but asserting that activities are consistent with federal law. The document requires vendors to complete detailed contact information, including name, address, FEIN, phone number, and email, and mandates signature by an authorized representative. The New Jersey Treasury Department reserves the right to take appropriate actions, including sanctions, seeking compliance, recovering damages, declaring default, or seeking debarment if a vendor's certification is found to violate the law.
