About this file

This is a demolition signature page for a planned residential demolition project in Detroit, Michigan, specifically for the property at 21540 Lyndon (Group 9.3.26). The document establishes the submission requirements and attestations necessary for contractors bidding on this demolition work. Bidders must submit their proposals under their full legal business name, provide their Federal Employer Identification Number, and disclose total employees and the number of employees residing in Detroit. A critical requirement is that all bidders must physically visit the demolition site(s) to inspect and review property conditions before submitting a bid, and submit a date-stamped photograph of the visited property with their bid response. The authorized company officer or individual with documented authority to commit the firm must sign the submission, and all signatories must be listed on the company's Certificate of Authority.

Contract assignments are prohibited without prior written approval from the Chief Procurement Officer, the Chief Financial Officer, and potentially the City Council. The Purchase Order will be issued and payment made only in the name of the bidding entity as submitted on the signature page. Contractors must provide contact information including email, telephone number, and cell phone number for an alternate company contact to facilitate communication throughout the bidding and execution process.
