About this file
This is a signature page for a demolition contract opportunity issued by the City of Detroit, Michigan for planned residential demolition services. The document outlines bidding requirements and submission procedures for contractors interested in demolishing the property located at 21610 Lyndon. Contractors must submit bids under their full legal name and provide their Federal Employer ID number, total employee count, and the number of employees residing in Detroit. A critical requirement for all bidders is that they must physically visit and inspect the demolition site(s) before submitting their bid proposal. Bidders are required to submit a date-stamped photograph of the property they visited with their bid response; this photograph must be different from any photographs provided in the Notice of Emergency Ordered Demolition.
The signature page requires authorization from a firm officer or an individual with documented authority to commit the firm to the bid, with the signatory's name listed on the Certificate of Authority. Bidders must provide contact information including email address, telephone number, and cell phone number. The document notes that Purchase Orders and payments will be issued only in the name of the bidding entity, and any assignment of the contract or monies due requires prior approval from the Chief Procurement Officer, Chief Financial Officer, and potentially the City Council. Contractors should contact the Contracting and Procurement Specialist for proper assignment procedures.
