About this file

This is a Certificate of Eligibility and Debarment document required by the State of Maine for a contract opportunity involving a Veeder Root Model TLS-300c. The document serves as a certification requirement that bidders must complete and submit as part of their proposal process. The certificate requires the bidding company to confirm its eligibility to participate in public transactions by attesting to several conditions, including verification that the company is not currently debarred, suspended, proposed for debarment, or declared ineligible by any Federal Department or Agency.

The bidder must certify that it has not been convicted of or had civil judgment rendered against it for fraud, criminal offenses, anti-trust violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property within the three-year period preceding the proposal submission. Additionally, the bidder must confirm that it is not presently indicted or criminally or civilly charged with any such offenses and has not had any public transactions terminated by default within the three-year period. The bidder is required to sign and date the certificate with printed name and title, and must promptly notify the State of Maine if any information contradicts the stated certifications. If unable to certify to all statements, the bidder must provide written explanation to the State of Maine.
