About this file

This is a Debarment and Suspension Certification form required by CTCLUSI (Connecticut Cooperative Library Consortium and Universities for Services and Library Information) for the Membrane Roof Repair/Replacement project at 149 W 12th Ave, Eugene, Oregon. The certification document establishes compliance requirements that contractors must meet prior to contract award.

The contractor must certify that it and its principals are not currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from federal covered transactions. Additionally, contractors must confirm they have not been convicted of or had civil judgment rendered against them for fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property within the three-year period preceding the contract. Contractors must also verify they are not presently indicted or criminally or civilly charged with any such offenses and have not had public transactions terminated for cause or default within the preceding three-year period. The contractor agrees to provide immediate written notice to CTCLUSI if the certification becomes erroneous and must include identical debarment and suspension language in all subcontracts and lower-tier covered transactions. Breach of this certification may result in suspension, termination, or debarment proceedings initiated by CTCLUSI.
