About this file

This is a Small Business Participation Plan (SBPP) form issued by the City of Phoenix Public Transit Department in compliance with the Disadvantaged Business Enterprise - Neutral (DBE-N) program requirements established under 49 CFR Part 26 by the United States Department of Transportation. The document applies to the Worker's Compensation Third Party Administration (TPA) Claims Management Services contract, which is a five-year agreement commencing on July 1, 2027, with no extension options. The contractor is required to submit an updated SBPP annually on the anniversary date of contract execution and must track all small business participation occurring through procurements, joint ventures, goods and services arrangements, and other business relationships. The plan requires contractors to demonstrate good faith efforts in promoting small business participation by contacting the PTD Compliance Specialist, identifying verified small business suppliers, conducting proactive outreach to small businesses, sending solicitation letters to potential small business partners, and maintaining detailed logs of all contacts and responses.

The contractor must document all small business participation monthly in the City of Phoenix Certification and Compliance System at phoenix.gob2g.com and provide supporting documentation with each annual SBPP submission. The form requires contractors to list current small businesses in use by category (subcontractors, joint venture partners, suppliers, or other arrangements), report total amounts paid during the past year, identify future opportunities for small business participation with estimated timeframes, and designate a contract liaison responsible for submitting the plan and other required reports. No specific pricing terms, set-asides, or funding amounts are detailed in this SBPP template, as it serves as a procedural framework for compliance with federal transit administration requirements on FTA-assisted projects.
