About this file
This document is a Darfur Contracting Act Certification form required by the State of California Department of General Services (DGS) under Public Contract Code Sections 10475-10481. The certification applies to companies that have had business activities or operations outside the United States within the previous three years and wish to bid on or submit proposals for State of California contracts.
The form provides two options for compliance: Option #1 allows companies to certify under penalty of perjury that they are not a scrutinized company as defined by Public Contract Code 10476. Option #2 provides a pathway for scrutinized companies that have received written permission from DGS to bid on or submit proposals, requiring them to include documentation of this permission with their submission. The form requires company identification information including company name, Federal ID Number, authorized signatures, and execution details regardless of which option is selected.
